NOTICE!!! Effective January
1, 2009, we will only
accept the following insurance plans:
- Cigna (HMO and PPO),
- Aetna (HMO and PPO),
- Medicare,
- Tricare
- Blue Cross and Blue Shield (all plans including Anthem)
As always,
we encourage you to inquire about our participation with your insurance
company regularly, when scheduling appointments.

Insurance plans we are accepting now:
Effective September 1, 2008, we will no longer be participating
providers with Beechstreet Insurance Company. We apologize
for any inconvenience this may cause. As always, we will
gladly see you on a fee for service basis and provide you
with all necessary documentation to submit your claims on
your own. |
