Forms and Special Letter

Administrative Fees for Completing the following form:

A minimum administrative fee of $25 - $50 (depending on the complexity) will be collected for the completion of the following forms. These forms and visits include, but are not limited to:

• Disability Insurance
• Assisted Living Form
• State Department/Peace Corps -Adoption
• College/Grad School
• Life Insurance
• Travel Visit Consults (50)

An administrative fee of $10 will be collected for the completion of each and every one of the following forms. These forms include, but are not limited to:

• School/Day Care/Camp/Sports
• DMV Handicap Certification -PPD

Please allow 5 – 10 business days for the forms to be completed. You will be asked to pay for the services when the request is made. Rates may vary based on the complexity of the form.
