Medical records may be requested from our office for your personal use. Please complete the appropriate form and we will process your request within 7 - 10 business days. Our fax number is 703-243-1151 if you want to fax us your request.

Please note our charges for medical records are as follows and due prior to our releasing your records:
$10 processing fee
$.50 per page up to 50 pages
$.25 for each page above 50 and
Postage fees

If you need your records transferred to another physician, please complete the form and we will forward your records to that physician.
