Preparation for In-Person Visits

If you have a fever, cough and shortness of breath or have had direct exposure to a person diagnosed with Coronavirus illness then please do not walk into the office. Call our mainline at (703) 243-1300 and we will talk to you on the phone about your symptoms and what we can do to help.

HOW TO PREPARE FOR YOUR IN PERSON VISIT

FOR ALL PATIENTS: 

  • We ask that you call us ahead of time if you are experiencing any symptoms such as fever, coughing, shortness of breath or difficulty breathing or have had direct exposure to a person diagnosed with Coronavirus illness.
  • Please log onto the patient portal and update your demographics and upload a copy of your insurance card and driver’s license 24 hours before your appointment.
  • Please complete all forms and bring completed forms with you to your appointment.
  • If you have school forms that need to be completed please bring them with you. 
  • Wear your mask to the appointment.  Children 2 years old and under do not need to wear a mask.
  • No visitors to the office other than the patient (one parent is allowed to accompany a child).  Children who do not have an appointment will not be allowed, so please plan ahead. 

Initial Newborn Visits:

  • Please follow the instructions for ALL PATIENTS
  • Complete all demographics and provide us with the hospital paperwork BEFORE the appointment via the Patient Portal

18 Month Well Visits:

  • Please follow the instructions for ALL PATIENTS
  • Complete the MCHAT Form and bring it with you

THE DAY OF YOUR APPOINTMENT

FOR ALL PATIENTS:

  • Wear your mask to the appointment.  Children 2 years old and under do not need to wear a mask.
  • Please arrive 10 minutes before your scheduled appointment time and wait in your car until our Medical Assistant calls you on your mobile phone.
  •  When the Medical Assistant asks you to come up, please check in with our concierge in the building lobby and let them know you have an appointment and they will key you up to the 3rd floor.
  • The Medical Assistant will meet you by our entrance door and ask you a few screening questions, take your temperature and ask you to sanitize your hands with hand sanitizer and provide you with a proper mask if needed.
